Those who encounter it may find themselves lost in a torrent of their own regrets, leaving them vulnerable to its whispers—both intoxicating and terrifying.

Ethereal Reprisal. When a creature within 30 feet of it makes an attack against it, the creature must succeed on a DC 16 Wisdom saving throw or take 22 (4d10) psychic damage and be blinded until the end of their next turn.

Whispers of Regret. As a bonus action, it can target one creature within 60 feet that it can see. The target must succeed on a DC 16 Wisdom saving throw or be incapacitated until the end of its next turn, overwhelmed by vivid visions of their past regrets.

Actions

Multiattack. It makes two spectral touch attacks.

Spectral Touch. Melee Spell Attack: +10 to hit, reach 5 ft., one target. Hit: 23 (4d8 + 5) necrotic damage, and the target must succeed on a DC 16 Constitution saving throw or have their hit point maximum reduced by the amount of necrotic damage taken until the target finishes a long rest.

Lamentation’s Veil (Recharge 5-6). The entity envelops a 30-foot radius sphere centered on itself in a darkness that resembles a flowing mist of sorrow. Creatures within the area must make a DC 16 Constitution saving throw. On a failed save, they are blinded and take 14 (4d6) psychic damage at the start of their turn. On a successful save, they take half damage and are not blinded. The area is considered heavily obscured for the duration, which lasts for 1 minute. The entity can end this effect as a bonus action.

Rejuvenation. If it is destroyed, it can reform after 1d10 days, regaining all its hit points and returning to the location of its demise.
