Traits
Reflective Manifestation. When a creature within 30 feet of it makes an attack roll or saving throw, it can use its reaction to impose disadvantage on that roll, as its form twists and warps, reflecting the attacker’s insecurities.
Fearful Echo. When it takes damage, each enemy within 20 feet must succeed on a DC 15 Wisdom saving throw or take 3d6 psychic damage and be frightened until the end of their next turn.
Actions
Multiattack. The creature makes two attacks: one with its Corrupted Touch and one Illusory Lunge.
Corrupted Touch. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 21 (3d10 + 4) psychic damage. If the target is a creature, it must succeed on a DC 15 Wisdom saving throw or be incapacitated until the end of its next turn.
Illusory Lunge. The creature can target one creature it can see within 50 feet, teleporting to an unoccupied space within 5 feet of the target and making a Corrupted Touch attack as part of the same action. If the attack hits, the target must make a DC 15 Wisdom saving throw or be subjected to the effects of Fearful Echo.
Psychic Surge (Recharge 5-6). The creature unleashes a wave of psychic energy in a 30-foot cone. Each creature in that area must make a DC 17 Intelligence saving throw, taking 45 (10d8) psychic damage on a failed save, or half as much on a success. Creatures that fail the save also suffer the effects of being incapacitated until the end of their next turn.
